they do not feel comfortable hosting outside the United States. Whether it
be for the US government or large halthcare companies we are fiding they are
pulling implementations back to the US for reliability (infrastructure
related), corporate perceptions, as well as data security.

BizTalk is a great product, but any integration solution will have multiple points of failure that the operations staff needs to be mindful of. I would suggest monitoring your BizTalk solution.
